Binocular Fusion

Binocular fusion is the brain's ability to combine two slightly different images — one from each eye — into a single, cohesive, non-doubled view. It is the second of three degrees of binocular vision in Worth's classification, higher than mere simultaneous perception but lower than stereopsis. Fusion has two closely cooperating components. Motor fusion involves the work of the extraocular muscles — precise, continuous micro-adjustments of the position of both eyeballs that keep the two images close enough together for the brain to merge them; motor fusion is responsible for the phenomenon of convergence when viewing up close. Sensory fusion, on the other hand, involves the work of the brain's visual cortex — the actual perceptual merging of the two images into one, so that they are not perceived as two overlapping views, but as a single image.

Fusion has its range (amplitude) — the margin within which the images from both eyes can differ slightly in position before the brain can no longer merge them, resulting in double vision. This range is clinically assessed using prisms that gradually shift the image from one eye until the patient reports seeing double — this is one of the fundamental tests performed by an orthoptist when evaluating eye cooperation.

When does fusion fail? Weakened fusion can be associated with convergence insufficiency, uncorrected refractive errors differing between the eyes (anisometropia), and in more severe cases — with strabismus, where the brain sometimes "gives up" on attempting fusion and begins to suppress the image from one eye, which can lead to amblyopia. Symptoms of weakened fusion resemble those described with convergence: double vision when fatigued, difficulty reading, headaches.
