Vision Screening in Children
A screening test is a short examination carried out in all children of a given age group, whose purpose is not to establish a diagnosis but to single out those in whom a full assessment is worth carrying out.
In Poland, assessment of vision is part of the health check-ups — free preventive examinations carried out in successive years of a child's life, among others after the second, fourth, sixth and tenth year of life and in the teenage years. The scope differs with age: in a two-year-old, eye alignment is among the things examined, in older children visual acuity is checked, and around the tenth year of life colour vision as well.
It is worth understanding where the limits of such an examination lie.
A screening test does not replace an ophthalmological examination. It does not include an assessment of refractive error after instilling drops that paralyse accommodation, and without this the young eye can partly compensate for the error by its own effort — especially with hyperopia.
Not everything can be picked up by visual acuity alone. A child with anisometropia sees well with one eye and, in a careless examination, may not draw anyone's attention. This is why visual acuity is tested separately for each eye, with the other one covered.
Screening picks up risk, it does not establish a diagnosis. A referral after a check-up does not mean that something is wrong — it means it is worth looking more closely.
The ophthalmological community has for years called for vision prevention in the youngest children to be extended, among other things with an earlier examination in the first year of life. Until such changes come, most depends on the attentiveness of the parent and on whether the referral from the check-up is actually used.
